Independent Contractor Service Agreement

This Independent Contractor Agreement (Agreement) is entered into between Rogue Events, LLC (Company), and the independent contractor (Contractor), in consideration of the mutual promises made herein, as follows:

Term of Agreement

This Agreement will become effective on the date signed below and will continue to stay in effect until terminated by either party with written notice.

Services to be Rendered by Contractor

Contractor agrees to provide services in accordance with their skills and abilities.

Method of Performing Services

Contractor will determine the method, details, and means of performing the specific services, at the Contractor"s own expense. The Company may not control, direct, or otherwise supervise Contractors assistants or employees in the performance of those services.

Compensation

In consideration for the services to be performed by Contractor, Company agrees to pay Contractor the sum of the agreed upon hourly pay rate for the total number of hours worked within a reasonable time.

Tools and Instruments

Contractor will supply all tools, equipment, and supplies required to perform the services under this Agreement.

Workers Compensation

Contractor understands that they are performing services as a self employed individual and agrees to indemnify Company for any and all claims arising out of any injury, disability, or death of any of Contractor"s employees or agents.

Obligations of Company

Company agrees to meet the terms of all reasonable requests of Contractor necessary to the performance of Contractor"s duties under this Agreement.

Termination of Agreement

Notwithstanding any other provisions of this Agreement, either party hereto may terminate this Agreement at any time by giving 30 days written notice to the other party.

Partial Invalidity

If any provision of this Agreement is held by a court of competent jurisdiction to be invalid, void, or unenforceable, the remaining provisions shall nevertheless continue in full force without being impaired or invalidated in any way.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Arizona.

By signing below, I understand and acknowledge: that I am an independent contractor and that I am responsible for keeping track of my earnings; that I am not an employee, I am a non-employee providing services on an at-will basis; that I am responsible for paying my own taxes, and that no taxes are being withheld from my earnings, nor am I eligible for any employee benefits. I further understand that I will receive a 1099 form from the company and not a W2.

I, hereby authorize Rogue Events (the "Company") of 3104 E Camelback Rd #2132, Phoenix, Arizona 85016, and/or its agents to make investigation of my background, references, character, past employment, consumer reports, education, and criminal history record information which may be in any state or local files, including those maintained by both public and private organizations, and all public records, for the purpose of confirming the information contained on my application and/or obtaining other information which may be material to my qualifications for employment. A telephone facsimile (fax) or xerographic copy of this consent shall be considered as valid as the original consent.

I hereby consent to the Company"s verification of all the information I have provided on my application form. I also agree to execute as a condition of employment or a condition of continued employment any additional written authorization necessary for the Company to obtain access to and copies of records pertaining to this information. I also hereby authorize the Company"s access to any medical histories or records pertaining to me (and any other individuals who due to my employment may be covered by any Company medical or other insurance program). With regard to the foregoing disclosures, I hereby agree to release any person, company, or other entity from any and all causes of action that otherwise might arise from supplying the Company with information it may request pursuant to this release. I understand that any false answers or statements, or misrepresentations by omission, made by me on this application or any related document, will be sufficient for rejection of my application or for my immediate discharge should such falsifications or misrepresentations be discovered after I am employed.

Staff Drug Testing Consent
I, Applicant Name, of Applicant Address, hereby agree, upon a request made under the drug testing policy of Rogue Events, LLC of 3143 E Camelback Rd #2132, Phoenix, AZ 85016, to submit to a drug test and to furnish a sample of my urine, breath, and/or blood for analysis. I understand and agree that if I at any time refuse to submit to a drug test under Company policy, or if I otherwise fail to cooperate with the testing procedures, I will be subject to immediate termination. I further authorize and give full permission to have the Company and/or its Company physician send the specimen or specimens so collected to a laboratory for a screening test for the presence of any prohibited substances under the policy, and for the laboratory or other testing facility to release any and all documentation relating to such test to the Company and/or to any governmental entity involved in a legal proceeding or investigation connected with the test. Finally, I authorize the Company to disclose any documentation relating to such test to any governmental entity involved in a legal proceeding or investigation connected with the test. I will hold harmless the Company, Company physician, and any testing laboratory the Company might use, meaning that I will not sue or hold responsible such parties for any alleged harm to me that might result from such testing, including loss of employment or any other kind of adverse job action that might arise as a result of the drug test, even if a Company or laboratory representative makes an error in the administration or analysis of the test or the reporting of the results. I will further hold harmless the Company, its Company physician, and any testing laboratory the Company might use for any alleged harm to me that might result from the release or use of information or documentation relating to the drug test, as long as the release or use of the information is within the scope of this policy and the procedures as explained in the paragraph above. This policy and authorization have been explained to me in a language I understand, and I have been told that if I have any questions about the test or the policy, they will be answered. I understand that the Company will require a drug screen test under this policy whenever I am involved in an on-the-job accident or injury under circumstances that suggest possible involvement or influence of drugs in the accident or injury event.
